Phone Extension Issue

Hi, we have an OmniPCX Office system with Alcatel 8038 and 8068 phones. Some users are seeing the old employee names of extensions when receiving an internal call or transfer. I think it's mostly on the transfers. We have had the system for 2 years, but it started doing this pretty recently.
When an employee leaves, I delete the mailbox rename the phone to the new employee, then recreate the mailbox so it's new and ready to be set up as the new employee..

Any assistance would be appreciated.

i think they have a programmed key with the old name...
