9700 web interface closes immediately

Our OS9700 switch immediately closes the web interface as soon as a user logs in. The swlog shows a successful login using http or https. As soon as you login it flashes the screen white and returns to the login page without any error.

I have disabled QOS and that doesn't make a difference. This started happening randomly and now it is pretty consistent. Only occasionally can you get in via the web interface. It is doing it on all user accounts including the admin.

Any help?
