If there is downtime for 4645 no issues but phones and other services should not stopped.

Cannot be done, as new binaries for the boards, these will reset so UA and Z cards and also NPRA (or T1) depending on ISDN type

I have seem sometimes our engineers to take new boards with binaries already loaded. This reduces the time.
Also sometimes they even take another pbx and put basic emergency services that can't go out and put in parallel to the actual pbx. Then they return after all upgrade have been done.
