I have a fix and yes it is quite odd.
Get one of the PSM cables that are used for connecting the PSM to the switch when the PSMs are rack mounted. Simply insert the cable into PSM slot 3 on the switch and remove.
Worked for us before on 10 separate incidents involving 6850 and 6850E various codes 644 and 646
