I was having this exact problem myself. I used this article in conjunction with a PDF I found on Alcatel's support site (8554_OS6250_flash_recoevry__uboot_based_file_transfer-1.pdf) and yes, they spelled recovery wrong on the PDF.
I encountered this problem after trying to administratively shut a port from the GUI interface of the switch. I realized after hitting OK, that the confirmation window said it was shutting down slot 1 (the entire switch).
What I did to fix it...
Booted to Miniboot and issued the sysNewfs "/flash" command and then reboot. I took a standard 1G flash drive and put the AoS image files onto it. The documentation was a little sketchy on the file structure it should be in and I kept getting this error:
[Miniboot]->Bulk Device mounted at 480 Mbps. With 1 Logical Units at node 2
USB Disaster Recovery Activated
sysUsbDisasterRecoveryTask: ERROR: /uflash/<target>/certified not found. Exiting.
Configure /uflash/<target>/certified and remount USB flash drive
After playing around with it a bit and using this document - download/file.php?id=1753 - I managed to land the correct directory structure and it properly started up a disaster recovery process.

Copy OK: 15510736 bytes copied
copying file /uflash/6450/certified/KFdiag.img -> /flash/certified/KFdiag.img

copying file /uflash/6450/certified/KFos.img -> /flash/certified/KFos.img

copying file /uflash/6450/certified/KFsecu.img -> /flash/certified/KFsecu.img

I'm now able to load the normal CMM. After wiping the flash and restoring the system, the Stack Manager was restored to defaults and the switch came back on functioning normally. Hopefully my addition to the thread may save someone else a few hours of time if they do the same thing to their switch.
