With t3 trace - you see second incoming call (to 3100) and have immediate disconnect with reason "User busy"?
What is your PBX release exactly?

Hello guys, sorry for the late reply.
@ Vad you were right T3 traces showed Disconnect/Busy. It is a OXE R7.0-f4.401-13

We got the customer to authorize a maintenance window and restarted the OXE last weekend.
I'm a bit frustrated to say that after de reboot the problem was solved and now all multiline keys get busy.
