The time on out phones drifts about -1 minute per week.
If I go to swinst to the NTP server configuration I can use
the "Instant Synchronisation" to get the inhouse time reference
at 10.0.0.226 and all works fine.

Then I start the NTP server and it seems to start OK but
the phone time will drift again like the NTP server is not
syncing up.

When I set up the NTP server I added the address then took
the defaults for the rest.

BTW I tried the PRI time sync and that doesn't work either.
Using t3 I see the connect message but no data/time.

Lets start from the end.
OXE/4400 can take time from PRA only on answered outgoing calls, of course other end should provide time message with CONNECT.
Exist specific part in documentation dedicated to NTP.
