Check the set type configuration on OXE. Also if auto answer is enabled or not

The phones type are 4018 & 4039 and auto answer is disable.

Yes, but in OXE, are they configured like those? Sometimes a bad configuration sets a 4068 as a 4038 for example and even when phone works it behaves and looks bad

I think this is not possible.
Speaker led:
- OFF - phone loudspeaker switched off
- ON - hands-free On
- Flash - phone loudspeaker and handset loudspeaker work at the same time.
Ask your customer - may be enough to customise ring melody (in this case user understand - which telephone ring)?

I have tested this on two customers and never happens. Asked users if they ever saw that button blink and answer was never. So, I agree again with vad. If you say it is possible and we after tested and vad by huge experience say is not, then, something must be missing from your side. Maybe was another configuration. Maybe they used headsets. But not by default.

May be for attendant - we have some specific behaviour. F.e. when attendant dial user number (but not calls) - user "reserved" for attendant and so on.
But about usual calls - I never hear about specific management for speaker LED.
