For Alcatel phone user encountered below problem, is it a configuration problem, or it is system design to press "DTMF" button before accessing IVR system.

Is your IVR attached to your Alcatel PBX? Or just a user calls an external IVR and wants to send MF? If external its normal he has to press the MF button in order to send tones. If internal activate VSI Transparency in your IVR ports.
