cannot access 4645 voicemail / mailbox

Hi guys,
I have a problem at our customer site.
Their system is using forward on no reply or busy on dynamic state user to voice mail (on all users).
The problem is right now we cannot access the voicemail even when we use voice mail access/consultation prefix directly from a phone.
The consequence is when an extension recieve an external call and the are not picking up, automatically it's forwarded to voice mail.
but the voice mail ports are sort of all busy even there is no one using the voice mail ports.
so the result is the external caller hear music on hold for a few moments and then it got forwarded to the operator,
of course they complaint about it so badly.
the operator set always got those call, where it's not suppose to happen.
also happen when external caller directly go to operator, and ask to be transfered to certain extention,
when that extention is not picked up, heard music on hold and then the call return to the operator again.
please do advice.
