Yes, I disabled digest authentication. It is not an authentication issue because I can make a call from an Alcatel 4068 phone to a SIP phone but not vice versa.

The authentication is only for SIP clients.
You can set it up for incoming, outgoing, or both calls.
I think by defaul, it is setup for outgoing calls.. But if you said you tries that..

What did you put for:

Sip / Sip Proxy / Minimal Authentication Method

Also, in your client, what does the log says ?

Registration: 503 Service unavailable
Called Failed: Service unavailable

or something else ?

I did not really look deeply at your logs (lazy I am), but if it s still not working, I'll check it out. What client are you using ?

503 Service Unavailable

The server is temporarily unable to process the request due to a
temporary overloading or maintenance of the server. The server MAY
indicate when the client should retry the request in a Retry-After
header field. If no Retry-After is given, the client MUST act as if
it had received a 500 (Server Internal Error) response.

A client (proxy or UAC) receiving a 503 (Service Unavailable) SHOULD
attempt to forward the request to an alternate server. It SHOULD NOT
forward any other requests to that server for the duration specified
in the Retry-After header field, if present.

Servers MAY refuse the connection or drop the request instead of
responding with 503 (Service Unavailable).

In the client log I get 503 Service Unavailable

Frank, thanks for helping debug this;

I have used different SIP clients such as GrandStream, ExpressTalk, Xlite, SoftPhone and I always get the same error in client's end:
"503 Service Unavailable"

SIP proxy settings are as below:

* Sip / Sip Proxy / Minimal Authentication Method = None
* "Only Incoming Authenticated Calls" is NOT checked. Although I have tested the other case (this setting is checked) and then restarted the sip engine but get the same error.

*SIP Initial time-out - 500
* SIP Timer T2 - 4000
* SIP Connection duration - 180000
* Recursive search - Unchecked
* Authentication Realm - left blank but also have tried to put the ip address of the CS (SIP Gateway) with same result

I should have otherwise I wouldn't be able to call a SIP client from an analog or Alcatel IP Phone. Also, I was able to create a SIP trunk.

Am not sure if these two arguments prove that I indeed have a license for a SIP trunk or not?

Yes, you have a valid license.
I still have the same issue, i'm going to do an upgrade to the OXE version beacuse we have found a couple of issues with OTUC too, also with the SIP.
Will let you know after the upgrade.
