Hi,
I hope the below applies to your omnistack.
Option 4 should help you. Console Cable needed...

Alright, its figured out.
Downgrading and upgrading the code via the console, corrupts the configuration, there by allowing to login to the switch with the default username and password.

Re: how to reset console login password for Omnistack 6124
Should be:
From Technical Document:
To reset an OmniStack to factory defaults - you'll need to connect via a console serial cable, then:
hit any key when prompted by boot process immediately after power-up to get to boot/diagnostic menu
enter 'c'<return> (don't enter the quote marks!)
enter <return> until line displays: "Boot Flags :0xb" then enter '0x1000'<return> at end of line
enter <return> until back at the "[Boot]:" prompt
enter '@'<return> to boot
system is now reset to factory defaults
Login as 'admin', initial password 'switch'
enter 'pw admin', to set a more suitable password
Other:
Stop Auto boot
With L list all files in NVRAM
Delete mpm.cfg and mpm.cnf with the command R
I think it will reset the whole switch, so config is gone as well. If you have another user (smmp?) use this for login and reset the password for admin
