shelf not initializing properly

Hi all,

I have configured R9.1 patch 25b on appliance server, i have created 1 ACT14 shelf and created two boards in it IOIP(NTIP2) and eZ32, but i am unable to initialize eZ32, i have replaced this EZ32 with with eUA and also tried to initialize NPRAE in other slots but failed to do so, following are the incidents which i get everytime i try to initialize my shelf ACT14.
