CCA on-hold problem

Hi everybody. This the problem: when I use the button in the CCA bar to put on-hold somebody, I can't return to the communication and the person who called me continues listening to the music on-hold. I have to hook-on and the communication became lost. The agents use analog sets.
I hope someone know how to solve the problem.
