Also on the sip dictionary, check if u have the dns name on the SIP URL domain.

Can you describe the topology of the call? Are there any compressors involved in the call or a direct RTP. Run a wireshark on that phone and try to replicate the issue..

I assume you are using the NOE SIP Mode. If so, can you post the content of the sipconfis-MAC.txt of that Extension A and B.

I think you can reload the linux and check. Reload the linux and restore the mao. Ignore if you have already tried it

Could be a bad DSP/Compressor on the daughter board. The no audio calls might me hittting one of the bad compressors. Check which one with a compvisu on the coupler.
